As far as I recall in extranet you were able to mark/select if you want to include CVV to show in guest CC details.
No, you have to provide all the information.
When I was accepting credit cards I was able to select if I require a CVV info or not. Because my POS terminal didn't need CVV code for charging I didn't select this. Now I use "Payments by Booking.com" so don't have an insight to this information.

I selected CVV and all information. These people live in Italy, but the booking address is in Thailand.
It is also a possibility that on the time of booking they were currently just staying in Italy which doesn't neccesery mean that they actualy live there. For example a guest was already in Slovenia and booked our place (in Slovenia). The flag was shown slovenian although he was an italian.
Did you try pre-authorize the card? Or do you need a CVV to be able to pre-authorize/charge the card.
If you did select CVV in extranet as a required information it is indeed strange that the guest managed to book without CVV. I suggest that you contact your account manager if you have one in your area.
